I hereby acknowledge that by signing this waiver, I release Guardian BJJ, its employees, instructors, agents, and representatives from any liability for accidental injury, illness, or any other harm that I may incur as a result of participating in any physical activities, including but not limited to training, sparring, competitions, and other related events.

I hereby assume all risks associated with participation, including, but not limited to, injuries resulting from falls, contact with other participants, and the effects of the environment, and consent to participate in this program. I understand and accept that these risks may be caused by my own actions, the actions or inactions of others, or the negligence of Guardian BJJ, its employees, or representatives.

I agree to disclose to Guardian BJJ any physical limitations, disabilities, ailments, or impairments which may affect my ability to participate in this program. I also agree to abide by all safety instructions and guidelines provided by the organization.

I acknowledge that I have read and understand the Guardian BJJ concussion protocol statement. I agree to adhere to this protocol and to notify Guardian BJJ staff immediately if I suspect that I or another participant might have a concussion.View Cuncussion Protocol Statement

I further agree to indemnify and hold harmless Guardian BJJ, its employees, instructors, agents, and representatives from any claims, demands, or causes of action which may arise out of my participation in this program. If any provision of this waiver is found to be unenforceable or invalid, the remaining provisions shall be unaffected and continue in full force and effect. I have read and understand this waiver, and I voluntarily agree to its terms.
